Information For New Patients
Faith Family is not a walk-in clinic. All patients must have an appointment to receive care.
To be eligible for treatment, the patient or their legal spouse must be employed a minimum of 20 hours per week, or be a fulltime student (12 hours of course-work).
New patients need to bring the following information at the time of their first appointment:
- A current photo ID
- Proof of household income
- An IRS Form 1040 and/or
- Paycheck or unemployment compensation stubs for the most recent 30 days or
- Proof of your being a fulltime student
- Please also bring a list of your current medications
