Information for New Patients
The Faith Family Medical Clinic is an affordable family medical doctor's office that serves low-income working families that do not have any health insurance and cannot afford to pay the full costs of medical services. Our purpose is to become the family doctor for the greater Nashville area's hard working people that do have not access to health insurance. The Clinic: FFMC is a full-service family doctor's office located at 326 21st Avenue North at the corner of Murphy Avenue. It is one block south of Charlotte Avenue and one block north of Baptist Hospital. Dr. Tom Henderson, an experienced physician who is board certified in family medicine, heads the Clinic. FFMC is a non-profit organization supported by members of the community that want to help people get access to regular and affordable primary medical care. Local hospitals and physicians are also supporting FFMC through referral partnerships that allow the Clinic to refer its patients to them when the patient needs care beyond the capabilities of a family doctor's office. How To Get Care At The Clinic: FFMC is not a walk-in clinic; you must make an appointment to see the doctor. New patients should come by or call the Clinic and complete the registration process before making any appointments to see the doctor. FFMC is also not a free clinic; all patients will have to pay an affordable fee for each visit to the doctor. The fee for each visit ranges from $10 to $30 and the amount is based upon the family's annual gross income (before taxes) and the number of people in the family. Those applying for care must be receiving income in order to be a patient of the Clinic and cannot be eligible for any health insurance program, including TennCare or Medicare. FFMC provides care only to working individuals and families that have too much annual income to qualify for TennCare, do not have insurance with their jobs, and cannot afford private insurance. Children under age of 19 qualify for care at FFMC when their parent(s) meet the Clinic's guidelines. Those applying for care must bring with them a recent pay stub from their employer, their most recently filed tax return if they are claiming dependents, and a photo ID.
